OSSRC Academy Registration - 2021

IMPORTANT NOTICE:



If have previously completed the CONCUSSION TRAINING, and you must have the following information
to register for an Academy:

1. Lifetime USSF Game Count
2. Leagues worked (e.g. MOSSL, OSSL, NPL, etc.)
3. Complete game log (20 entries minimum - date, league, division, home and away teams, location).
